Registration and Forms

 

Registration forms and fees. To download the District letter detailing the information you will need to submit, please click here. If your child is new to District 90, you must visit the Administration Building (7776 Lake St.) to pick up both a residency packet and a registration packet. Waiver of Fees and/or Free Lunch/Milk Program. Families whose gross income is at or below Federal Income Eligibility Guidelines or families who receive public assistance from the Supplemental Nutrition Assistance Program/Temporary Assistance for Needy Families are eligible to receive a waiver of fees and/or free lunch/milk. Kindergarten Registration
To be eligible for kindergarten during the 2012-2013 school year, children must turn 5 on or before September 1, 2012. Families with older students currently attending District 90 schools may pick up registration packets for their new kindergartners in the Lincoln or Willard School offices. Families who are new to the District should pick up both a residency packet and a registration packet at the District Administration Building (7776 Lake St.).
In order to promote equity across the school community, District 90 does not use the order in which students are registered to make determinations about placement in AM or PM kindergarten sessions. 

Residency Boundaries
Kindergarten through 4th grade residents who live north of Chicago Avenue attend Willard School, 1250 Ashland Ave.

Kindergarten through 4th grade residents who live south of Chicago Avenue attend Lincoln School, 511 Park Ave.

All residents in grades 5 through 8 attend Roosevelt Middle School, 7560 Oak Ave.
